- Academic License Agreement (ALA)
- Addendum
- Please read and complete the ALA Annual Fee Worksheet
When you first join the Collaborative, you must fill-in all of the above documents.
If you are renewing, you only need to complete the Addendum and the ALA Annual Fee Worksheet.
